Vizi Beam 5RX Lamp Keeps Turning Off

I have a Vizi Beam 5RX that is not storing the command of Lamp ON. Every time I power it up, I have to go through the menu and turn the lamp on (except 1 time out of about 30, it kept the command). I also had the same fixture turn off the lamp when I was playing with gobos and/or the shutter. When I went to the menu, the lamp was ON. I had to power down the fixture and reset. What could possibly be going on?
