1/4" adapter in Haze Generator

I recently bought a Haze Generator on ebay and during the shipping the motor inside the unit hit the circut board and created a short where the 1/4 jack connects to the board.

Does anyone on here know where I can find a new jack to solder to the board?

The only thing it says on the jack is R4M
