Thanks for the reply. The image in this link is a scan of the only documentation that came with the light: http://davemonahan570.com/images/control-chart.jpg
There's no manufacturer written on the box nor the one-sheet owners manual as indicated in the link above. I think the light was made in China; that's all that came.
